Patient Information

Our mission is to provide you with the most advanced, comprehensive, and customized care for your sleep and wake needs.

During your initial visit, we will review your symptoms and your current sleep habits along with your medical history, current medications, and a physical exam.  Depending on your symptoms, we will discuss your differential diagnoses, the pathophysiology as it relates to your potential sleep issues, and discuss a customized sleep evaluation and treatment for you.  If you have any prior sleep studies or test results, we will review these together.

INITIAL VISIT

To schedule an appointment, please call us, email us, or submit a request via the patient portal.  During scheduling, please be prepared to provide your full name, contact information, and insurance information.  If you prefer to pay out-of-pocket, please indicate this at the time of the call.

In preparation for your appointment,  you will be asked to fill out some forms via the patient portal which will be reviewed prior to your visit.

Two days prior to your appointment, you will receive a reminder about the appointment.  During this notification, we will confirm whether we received your forms, review the information provided by your insurance company, discuss and collect payment information including your co-pay, and answer any last minute questions you have.

During the appointment, be prepared to talk about yourself!  The entire visit from start-to-finish will be face-to-face time with me, your sleep physician.

FOLLOW-UP VISITS

I believe that regular and continued follow-up visits are necessary to maintain your sleep health!  At each visit, we will continue to reevaluate your sleep health to ensure continued stability and optimization, review test results, and decide on management strategies together.
