New Patients: We are accepting new patients with a physician referral. If you are a new patient and would like to schedule an initial consultation, please ask your doctor to fax a referral form to our office (Fax: 312-955-8789).
TELEHEALTH VISITS: For 2023, we will continue to only schedule virtual Telehealth visits. We are using a service called Klara which can be used on a mobile phone or computer with a camera.
