Before Your First Visit

Thank you for choosing Chicago Sleep Health! Our mission is to provide the best sleep care for your individual needs.

In order to maximize your time with the doctor as much as possible, please follow the instructions before your appointment. This must be completed at least 72 business hours prior to your appointment time or it will be subject to cancellation.

  1. Please sign up for the patient portal here.
  2. Log into the patient portal to review and sign the three required new patient forms. To find these form, please go to the “My Health” tab on the left hand side. Locate where it says “Medical Forms” and click on the associated link. Review and electronically sign the following (note: you will not be able to sign when logged in on a mobile device):
    • Authorization for SureScripts
    • CSH Policy and Procedures
    • CSH HIPPA Acknowledgement
  3. Check with your insurance company to ensure that Dr. Margaret Park is in-network with your insurance plan at this office location.
  4. If you have had prior sleep care, please obtain and send copies of all past sleep studies to our office. They can be faxed to (312) 955-8789 or emailed to sleepteam@chicagosleephealth.com. We request these records to be sent prior to your appointment.

Rescheduling

If you are unable to make this appointment time, please notify us at least 24 business hours before your appointment so that we can help to reschedule you to a more convenient time and to avoid a $50 cancellation fee.

Directions

If using a GPS system, please use the address for Trump Tower (401 N Wabash Ave). If you enter our address into Google Maps or similar mapping software, it will direct you to the loading dock on the lower streets but the entrance to our building is on UPPER Wabash/Michigan/Wacker.
We are located next door to (not across from) Trump Tower and behind the new construction building (Renelle on the River).

  • From 90/94:  EXIT Ohio Street, RIGHT on Wabash Avenue.
  • From LAKE SHORE DRIVE:  EXIT Grand Avenue, LEFT on Wabash Avenue.
  • If WALKING, we are located in a building called River Plaza which is next door to Trump Tower, shares an outdoor plaza with Wrigley Building, and is directly behind a new construction building called Renelle on the River. We are directly south (up the hill) from Nordstrom at the Grand Avenue entrance.  You will find two restaurants located on the ground floor of our building: Bongiorno’s Cucina Italiana & Pizzeria and Beacon Tavern.

Parking

If you park at Trump International Hotel & Tower, we can provide parking validation (parking cost = $12). This can be provided at the time of your appointment.
